    ADMINISTRATIVE SUPERVISOR,
    INDIVIDUAL SUPPORTS-Mashpee

    Now seeking a Full Time (40 hours) Administrative Supervisor, Individual Supports who will work with an established team to provide supports to Learning Disabled community members who seek to enhance their independence and increase their social networks.

    Applicant must have excellent computer, organizational, and communication skills.  Experience with the Learning Disabled population preferred but will train the right candidate.  Proactive, positive team player approach mandatory!

    Responsibilities:

    • Maintain complete support records
    • Interface with support families
    • Interface with the Department of Developmental Services (DDS)
    • Develop, monitor and document support goals and progress in a timely fashion
    • Help to set department schedules
    • Work one on one with supports
    • Run team meetings
    • Experience with ISP’s a bonus but not mandatory

    LIFE COACH–Hyannis

    Now seeking a Full Time Life Coach/Case Manager to assist residents in fulfilling their potential by assisting them in managing their daily life skills – re: household management, shopping, menu planning, cooking, personal presentation and hygiene, appropriate behavioral and social skills, travel arrangements, medical appointments, entertainment, etc

    Applicant must be patient, outgoing and organized; have excellent verbal and written communication skills & strong computer skills. Must be a team player! Experience with Learning Disabled Individuals a plus; Education and or appropriate job experience desired.

    Responsibilities include:

    • Meets individually with assigned residents,  provide assessment of strengths and needs in all areas and writes appropriate  objectives
    • Assures adequate attention is given to skill and growth in all aspects of the resident’s life and helps prioritize areas of emphasis in conjunction with the Employment Team
    • Participates as requested in team-related services; staff meetings, housemate issues, case reviews with staff and/or family etc.
    • Plans, organizes and facilitates activity one night weekly
    • Develops and teaches groups and/or classes to address needs within the Individual Service Plans (ISP) as needed
    • Writes service reports as routine documentation after every session or significant communication with residents
    • Makes recommendations to meet needs of residents which cannot or should not be provided by the Life Coach: e.g. medication, therapy by licensed clinician, private tutor, housecleaner, etc.

    Employment Counselor—Hyannis

    Now seeking a Full-time Employment Counselor to work with our exceptional residents, community employers and employment staff to locate employment/volunteer opportunities and to match the right individual to the right position.     Responsibilities include:

    • Evaluating the resident for strengths and weaknesses in suitability for a positon
    • Working with resident through proper training and communication to ready them for a new position. Re: résumé preparedness, interviewing skills; work readiness skills, etc.
    • Work with the Job Coaches to ensure that residents and employers are being supported in a positive manner in the job site
    • Negotiating with potential employers to find a match position for our residents.
    • Document resident and employer related contacts and communications as needed
    • Additional responsibilities as per the full job description.

    Preferred Background:  Two years of successful experience in some or all areas of marketing, recruiting or vocational service; experience with adults with cognitive learning disabilities a bonus. High school diploma and some additional training or education ideal in the field is ideal.

    JOB COACH-Mashpee/Hyannis/Plymouth

    Now seeking a Full Time (30-40 hours weekly) Job Coach to provide on-site counseling, training and individual instruction to LIFE residents with developmental disabilities to help them reach their full potential in the workplace.

    Applicant must be patient; outgoing and organized; have excellent verbal and written skills; enjoy working with people of different age groups and backgrounds; have the ability to support residents to help them build confidence and independence in the workplace.

    Responsibilities

    • Will service the Hyannis, Mashpee and Plymouth areas
    • Work week is five days and WILL include Saturday; will NOT include Sunday
    • Must provide resident transportation to and from employment sites. Will need to use own vehicle from time to time (will be reimbursed for mileage)
    • Must prepare the resident for the job and supply on-site engaging support
    • Must address routine communication and problem
      resolution for residents and/or with employers on job sites
    • Must document the workplace experience
    • Must conduct routine weekly checks at employment
      sites as identified by Director
    • Must function as a TEAM PLAYER with the employment Services Team
    • All other related duties as assigned

    General statement of Duties and Responsibilities

    The Direct Care Professional provides direct care to physically challenged residents with learning disabilities, at Living Independently Forever, Inc., in order to maintain and improve their dignity, well-being, and safety, and to promote personal development, independence and enjoyment.

    Full Time & Part Time positions available.

    This position is eligible for a $250 Sign-on Bonus!

    The Direct Care Professional will support residents in becoming more independent in their daily living by:

    • Help plan, prepare and post a simple weekly menu.
    • Assist resident(s) with housekeeping, laundry, arrange clothing by outfit and other daily chores.
    • Assist resident(s) with self-administration of medications.
    • Assist with personal grooming and hygiene skills.
    • Assist resident(s) in preparation for their morning, evening or night routines.
    • Provide ongoing support and encouragement.
    • Review daily/weekly schedule.
    • Prepare meals and assist with transfer of items from place to place    (ie: hot and or breakable).
    • Accompany to grocery shopping and other necessary personal needs shopping.
    • Accompany resident(s) to a variety of fun activities and field trips.
    • Other duties as determined.

    Qualifications:

    • Experience and knowledge working with individuals with learning disabilities preferred.
    • Must be Medication Administration Program (MAP) Certified within six months of employment. May be obtained through LIFE.
    • CPR, AED, and First Aid certification – may be obtained after hire through LIFE.
    • Human Rights Certified – training may be obtained after hire through LIFE.
    • A valid driver’s license.
