    We are growing again, and LIFE is seeking a Full Time Case Manager/LIFE Coach to assist our wonderful residents with learning and cognitive disabilities to fulfill their independent potential by supporting them in managing their daily life skills. Re: money management, household management, shopping, menu planning, cooking, personal presentation and hygiene, appropriate behavioral and social skills, travel arrangements, transportation needs, medical appointments, entertainment, etc. Position hours are primarily normal business hours but do include 2 evening activities weekly and occasional weekend activities. (Ex: Rec Night; Bowling, Gym Night)

    Applicant must be patient, vibrant, energetic and be highly organized with excellent time management skills; must possess excellent verbal and written communication skills; must possess excellent money management skills, strong computer skills; and the ability to multi-task. Must be a team player!

    Experience with Learning Disabled population required. Social Service Work experience highly desired. Bachelor’s Degree OR 3+ years of directly related experience required.

    Responsibilities include:
    • Assist assigned residents with the management of their bills and finances–overall money management
    • Assist assigned residents with medical appointments and medication organization and proper documentation
    • Meets individually with assigned residents, provide assessment of strengths and needs in all areas, and writes appropriate objectives and goals (ISP’s)
    • Assures adequate attention is given to skill and growth in all aspects of the resident’s life and helps prioritize areas of emphasis in conjunction with the Employment Team
    • Participates in team-related services; DDS meetings, staff meetings, housemate issues, case reviews with staff and/or family etc.
    • Plans, organizes and facilitates resident activity two evenings weekly
    • Role models appropriate behavior and encourages residents in leading a healthy and active lifestyle
    • Writes service reports as routine documentation after every session or significant communication with residents
    • Makes recommendations to meet needs of residents which cannot or should not be provided by the Life Coach: example: medication, therapy by licensed clinician, private tutor, house cleaner, etc.
    • Focus all interactions towards helping the residents increase their independence and the quality of their day to day LIFE.

    Must have current certification or be willing to attend training through LIFE for CPR/AED/First Aid and any LIFE or DDS required/recommended training classes.

    Valid driver’s license required with acceptable driving record and dependable transportation; must be able to pass CORI and fingerprinting as per DDS.

    JOB COACH (Hyannis)

    LIFE–Living Independently Forever, Inc. is currently seeking a FT JOB COACH to provide counseling, training and individual instruction to our wonderful LIFE residents with learning disabilities to help them reach their full potential in the workplace.

    Position is 35-40 hours per week 8am-4pm M-F with an occasional Saturday.

    At Living Independently Forever, Inc. we are currently seeking a few caring and patient individuals to join the LIFE family as Part Time or Full-Time Direct Care Professionals to support our wonderful learning and cognitively disabled adults in their Home.

    This is done through instruction, guidance and by serving as a positive example to these active adults. You will  by helping them to maximize and maintain their independence in the community and their home by assisting with life skills –cooking, planning and reviewing daily/weekly schedules, cleaning, assisting in preparation for their morning, evening or night routines as needed, etc. and on errands and social activities in the community.

    A variety of shifts are available and if you are flexible, we can build you a full-time schedule: afternoon-evening (3pm-10PM-ish), weekends (9AM-9PM-ish) and overnights (9PM-9AM-ish)! The -ish means we will do our best to make it work for you.

    Applicant must be able to consistently project a positive and patient personality and provide ongoing support, interaction and encouragement to our clients, and work as part of the team to result in them living their best life!

     REQUIREMENTS:

    Direct Care Professional/Companion (Marshfield)

     We are currently seeking a caring and patient individual to join the LIFE family as a Companion/Direct Care Professional (DCP) providing instruction, support, and to serve as a positive role model to one of our active young adult clients with learning and cognitive disabilities. This position assists clients by helping them to maximize their independence in the community and their home by assisting with life skills –cooking, planning and reviewing daily/weekly schedules, cleaning, assisting in preparation for their morning, evening or night routines as needed, etc. and on errands and social activities in the community. This individual loves going out in the community–so a reliable vehicle is necessary.

    The position is 30 hours weekly

    (Monday 2pm-7pm, Tuesdays 2pm-7pm (-ish), Thursday 2pm-12am and Friday 2pm-12am). Hours are over 30 and therefore eligible for our BENEFITS SUITE.

    IF YOU ARE LOOKING FOR 40 hours that can be arranged working with additional clients in the Plymouth area. If you are looking for less hours–we may be able to make that work if you are the match for the position.
