    LIFE Coach (Case and Money Manager) (Mashpee)

    Approximately

    LIFE is growing again and we are now seeking an additional Full Time LIFE Coach/Case Manager to assist our wonderful residents in fulfilling their independent potential by assisting them in managing their daily life skills – re: household management, shopping, menu planning, cooking, personal presentation and hygiene, appropriate behavioral and social skills, travel arrangements, transportation needs, medical appointments, entertainment, etc. Position hours are primarily normal business hours but do include 2 evening activities weekly.

    Applicant must be must be patient, vibrant, and energetic with highly organized time management skills; must possess excellent verbal and written communication skills; must possess excellent money management skills, strong computer skills; and the ability to multi-task. Must be a team player!

    Experience with Learning Disabled population required. Social Service Work experience highly desired. Bachelor’s Degree OR 3+ years of directly related experience required.

    Responsibilities include:
    *Assist assigned residents with the management of their bills and finances–overall money management
    *Assist assigned residents with medical appointments and medication organization and proper documentation
    *Meets individually with assigned residents, provide assessment of strengths and needs in all areas, and writes appropriate objectives and goals (ISP’s)
    *Assures adequate attention is given to skill and growth in all aspects of the resident’s life and helps prioritize areas of emphasis in conjunction with the Employment Team
    *Participates in team-related services; DDS meetings, staff meetings, housemate issues, case reviews with staff and/or family etc.
    *Plans, organizes and facilitates resident activity two evenings weekly
    *Role models appropriate behavior and encourages residents in leading a healthy and active lifestyle
    *Writes service reports as routine documentation after every session or significant communication with residents
    *Makes recommendations to meet needs of residents which cannot or should not be provided by the Life Coach: example: medication, therapy by licensed clinician, private tutor, house cleaner, etc.
    *Focus all interactions towards helping the residents increase their independence and the quality of their day to day LIFE.

    Must have current certification or be willing to attend training through LIFE for CPR/AED/First Aid and any LIFE or DDS required/recommended training classes.

    Valid driver’s license required with acceptable driving record and dependable transportation; must be able to pass CORI and fingerprinting as per DDS.

    We are currently seeking a caring and patient individual to join the LIFE family as a Direct Care Professional (DCP) providing instruction, support, and to serve as a positive example to one of our active young adult clients with learning and cognitive disabilities.

    The DCP assists clients by helping them to maximize their independence in the community and their home by assisting with life skills –cooking, planning and reviewing daily/weekly schedules, cleaning, assisting in preparation for their morning, evening or night routines as needed, etc. and on errands and social activities in the community.

    Position is 22 hours weekly (Monday 2pm-6pm, Tuesdays 2pm-11pm, and Friday 2pm-11pm). If more hours are desired, that can be arranged with other responsibilities and/or clients in the Plymouth area.

    Applicant must project a positive and patient personality and provide ongoing support, interaction and encouragement to our clients.

    REQUIREMENTS:
    • Must be +18 years old

    JOB COACH  PLYMOUTH AREA

    Administrative Assistant Sandwich

The majority of the time this will be a shift where you can sleep from 10PM to 6AM–but must be available if a need arises.

    GROUP HOME MANAGER for Adults with Learning Disabilities (Hyannis)

    We are currently seeking a caring and experienced individual to join the LIFE family as a GROUP HOME MANAGER for our brand new Hyannis home for 4 adults with learning and cognitive disabilities as they begin to enter the aging stage of life. Most of these residents have been with the LIFE program for a long time.

    The Group Home Manager will oversee the clinical, financial and administrative needs of the EXCEPTIONAL residents we serve. They will also work with the LIFE Coaches from the Hyannis LIFE Community who have supported many of these residents for a long time for a seamless transition to their new home.

    This position will also ensure that proper standards and regulations are followed, and that all quality standards for the residents in the group home are being met. Additional responsibilities include managing, scheduling and training staff; working with appropriate agencies to assist and support our residents; working with the staff to assist individuals in developing the skills they need to function as independently as possible within the community for self-fulfillment and happiness.

    In addition, the Group Home Manager, along with the staff, works with the residents providing instruction, support, and to serve as a positive example to our residents inside the group home residence, and on errands, appointments and activities in the community. Applicant must be able to project a positive personality and provide ongoing support, interaction, kindness and encouragement. The GH Manager must also be able to work as part of the Group Home Management Team for the benefit of all of the LIFE residents.

    REQUIREMENTS:

    • Education or experience with Learning Disabled Population Required. Management Experience a plus.
    • Experience with aging population a plus.
    • Work schedule flexibility–Majority of shifts are Monday-Friday 8AM-4PM–however a few evening and weekend shifts will be required monthly so you can manage the entire team (or in a pinch for coverage)
    • Prior work in a Group Home a plus
    • Strong organizational and administrative skills required
    • Good computer skills needed
    • Must be able to effectively communicate in both verbal and written form with coworkers, supported individuals, families and relative agencies
