Current Openings:

Case Manager/LIFE Coach (Plymouth)

$1,000 Sign-on Bonus ($500 at 3 months, $500 at 6 months – must maintain trainings and average 20 hours weekly.)

$18.00 per hour, full-time position.

The LIFE community based in Plymouth is seeking a full-time Case and Money Manager (LIFE Coach) to assist our residents in fulfilling their independent potential by assisting them in managing their daily life skills: activities in the community, household management, menu planning and shopping, meal preparation, personal presentation and hygiene, appropriate behavioral and social skills, routine fitness, transportation needs, medical appointments, money management and access to recreational entertainment, etc.

Qualifications:

Patience, Kindness and Dependability and the ability to support these wonderful clients to have productive and happy lives, lived as independently as possible!

Experience and/or education with our population is needed (professional, personal, or volunteer).

This full-time position is primarily regular weekday hours; however, one early weekday evening (no overnights) and a rare half-day on either a Saturday or Sunday rotation is required.

Flexibility is available depending on how the schedule needs of our individuals are met.

Your skills:

  • Must have good organizational skills.
  • Must have excellent verbal and written communication skills and good computer skills.
  • Must be a team player with the ability to work with the entire team to the benefit of our clients.
  • Experience with Human Services and/or Learning Disabilities is required. If you have good relationship skills, we will train you! (Special Olympics experience and personal experience is valued.)

Responsibilities include:

  • Assist, organize, and facilitate activities as assigned.
  •  Assist the residents with meeting their goals and objectives in apartment maintenance, personal health maintenance, nutrition, fitness and social skills development.
  •  Assist the residents with grocery shopping and preparing healthy meals safely.
  •  Participate in team-related services; staff meetings, resident/roommate issues, resident case reviews with staff and/or family, etc.
  • Complete routine documentation after every session or significant communication with residents.
  • Make recommendations to meet the needs of residents that are beyond the scope of the LIFE Coach (i.e., medication adjustments, therapy by licensed clinician, private tutor, house cleaner, etc.).
  • Maintain sanitary conditions of workspaces and vehicles; complies with LIFE guidelines to prevent the spread of COVID-19 (i.e. uses personal protective equipment, social distancing, etc.).

Must have a valid driver’s license with acceptable driving record and dependable transportation. Must be able to pass CORI and fingerprinting background checks through our governing agency (DDS) and DPPC.

Must have current certification in CPR/AED/First Aid or be willing to attend training through LIFE.

Group Home Direct Care Professional

 GROUP HOME DIRECT CARE PROFESSIONAL

for adults with intellectual disabilities

$16.50 per hour

IDEAL SCHEDULE NEEDS:

E. FALMOUTH: Tuesday-Friday 2PM – 9PM and Saturday 9AM – 9PM

CENTERVILLE: Friday 4PM – 9PM, Saturday 3PM – 9PM, and Sunday 9AM – 9PM OR 1PM – 9PM

Note: On Sunday shifts we pay a differential of $1 per hour for hours worked between 9AM and 9PM

We can be flexible with your scheduling needs.

Most of these homes have active clients who engage in many activities they cannot wait to get back to as the pandemic continues to stabilize. If you would like either a part-time or full-time position–we can work that out for you.

All positions +30 hours weekly enjoy an EXCELLENT benefits package including:

Positions of +20 hours also have Holidays and PTO and access to some of our other benefit programs as well.

We are seeking Sunday staff with the added incentive that working on Sunday (9AM-9PM) is an additional $1 per hour! We will work with you to complete a schedule that works for your life and ours – full-time or part-time.

  • Applicant must be able to project a positive and patient personality and provide ongoing support, interaction, and encouragement to the individuals we support.

The support of these active and heart-warming residents is done through:

    • Instruction, guidance and by serving as a positive example to these active adults
    • Taking them on community outings and/or planning in-home activities to help them have more FUN!
    • Utilizing your gifts to plan activities that will excite you and the individuals we support
    • Assisting them to maximize and maintain their independence in their home and in the community by assisting with life skills –cooking, planning, and reviewing daily/weekly schedules, cleaning
    • Assisting in preparation for their daily routines as needed, etc. and on errands and social activities in the community or virtually.
    • Role modelling and maintaining sanitary conditions of workspaces and vehicles and complying with LIFE guidelines to prevent the spread of COVID-19
    • Dependability on meeting scheduled shifts

REQUIREMENTS:
• Must be +18 years old and up to infinity. The individuals we support learn from anyone of any age who wants to support them
• Valid driver’s license required with acceptable driving record and reliable vehicle (mileage is reimbursed on the rare occasion when driving your personal vehicle is needed.)
• Must be able to effectively communicate in both verbal and written form with supported individuals, co-workers and parents
• Must be able to manage an individual e-mail account and basic computer entries
• Must be willing to attend required training classes including CPR/First Aid and MAP Training (provided by LIFE)
• Per the state of MA DDS, MUST be able to pass CORI, DPPC and Fingerprint check and have an MVR record acceptable to LIFE’s Auto Insurance carrier.

Shared Living Provider

Shared Living Provider for cognitively disabled adults with intellectual disabilities and autism. Must be available to re-locate to Cape Cod, MA. 

Join our growing team of compassionate and reliable Providers to support the wonderful individuals we serve at LIFE – Living Independently Forever.

Shared Living is an arrangement available to qualified adults that is funded by The Department of Developmental Services (DDS) and supported and managed by LIFE.

We are currently seeking a compassionate and reliable Shared Living Provider to oversee the support of a young man with some medical challenges. (Any necessary training LIFE will provide.)

The Shared Living Provider lives with this individual.

In this crucial role you will:

  • Help our participant to live a full and meaningful life.
  • Assist in planning, food shopping, prep, and cooking with participant
  • Prepare paperwork for and attend appointments as needed
  • Transport participant to appointments, and activities
  • Continuously attend to medical needs with in home needs, doctors appointments, proper documentation, and if necessary, medication under the Medication Administration Program guidelines.
  • Be able to provide hands on ADL (Adult Daily Living) as needed.
  • Ability to manage an email account and other basic computer entry programs.
  • Maintain confidential files and complete documentation as required.

 

Qualifications:

• First Aid & CPR Certified (LIFE will provide training)
• Valid Driver’s License w/ reliable transportation (vehicle)
• COVID guidelines followed – PPE, etc. as required and needed (mask in home not required in most circumstances).

  • We are currently seeking 2 Full-Time or Part-Time Job Coaches to provide counseling, training, and individual instruction to the wonderful LIFE individuals we support to help them reach their full potential through workplace training on-site that are located throughout the Mid-Upper Cape Cod area.
  • Must live in or around the Cape Cod, MA area.
  • The full-time position is primarily Monday-Friday 9AM-5PM; with occasional Saturday or evening availability. This full-time position (35-40 hours) offers an exceptional benefits package including BCBS Medical and Dental Insurance.
  • The part-time position is shorter day shifts – Saturday may be needed (occasionally) for this position.
  • The Job Coach will be working with different individuals at different work locations to help them be their best.
  • Note: If you are the right person for one of these openings, we will do our best to make your schedule and the individuals we support schedule work.

  • Experience with the population we work with is excellent, but we will train the right candidate (Personal experience and volunteer work counts!)
  • Must be able to maintain professionalism in all workplaces
  • Must be willing to attend training through LIFE for CPR/First Aid (great if you already have) and be willing to complete any LIFE or DDS required/recommended training classes
  • Per the state of MA DDS, must be able to pass CORI, DPPC, and Fingerprint check and have an MVR record acceptable to LIFE’s auto insurance carrier.
