Community Direct Care Professional (Mid Cape Area)

At LIFE (Living Independently Forever, Inc.) we are currently seeking a caring and patient individual to join the LIFE family as a Direct Care Professional (DCP) providing instruction, support, and to serve as a positive example to our active young adult clients with learning and cognitive disabilities. 
 
The DCP assists clients by helping them to maximize their independence in the community and their own home by assisting with life skills –cooking, planning and reviewing daily/weekly schedules, cleaning, assisting in preparation for their morning, evening or night routines as needed, etc. Attending to errands and SOCIAL ACTIVITIES in the community is a key part of the position.
 
You will be working in a variety of locations between the mid-and upper Cape Cod. Mileage will be reimbursed.
A valid driver’s license is required with an acceptable driving record and reliable insured vehicle (mileage is reimbursed)
 
The position is 35-40 hours weekly (Monday-Friday 12PM-8PM). Occasional weekend shifts for special activities may be needed. PT shifts may be available.
 
The applicant must project a positive and patient personality and provide ongoing support, interaction and encouragement to our clients. Our staff are role models to our clients at all times.

REQUIREMENTS:

  • Must be +18 years old
  • Kindness and patience!!!
  • Must be able to effectively communicate in both verbal and written form with supported individuals, parents and coworkers
  • Must have basic computer skills
  • Experience with Learning Disabled population a plus, but WE WILL TRAIN THE RIGHT PERSON
  • Must be able to pass a CORI and fingerprint background check
  • Must be willing to attend required training classes (LIFE will provide)

LIFE COACH (Case and Money Manager) (Plymouth-Pine Hills)

LIFE is growing again and we are now seeking a Full Time Life Coach/Case Manager to assist our wonderful residents in fulfilling their independent potential by assisting them in managing their daily life skills – re: household management, shopping, menu planning, cooking, personal presentation and hygiene, appropriate behavioral and social skills, travel arrangements, transportation needs, medical appointments, entertainment, etc.

The position hours are 40 hours a week and include one weeknight evening for an activity and an occasional Sunday activity about every two months.  The position comes with EXCELLENT BENEFITS & a Great team who are highly motivated!

Applicant must be patient, outgoing and organized; have excellent verbal and written communication skills; strong computer skills. Must be a team player!

Experience with Learning Disabled Individual population a plus. Social Services work experience highly desired. Bachelor’s Degree OR closely related experience required.

Responsibilities include:

  • Assists with assigned residents with the management of their bills and finances

  • Meets individually with assigned residents, provide an assessment of strengths and needs in all areas, and writes appropriate objectives and goals

  • Assures adequate attention is given to skill and growth in all aspects of the resident’s life and helps prioritize areas of emphasis in conjunction with the Employment Team

  • Participates as requested in team-related services; documentation of case time, staff meetings, housemate issues, case reviews with staff and/or family etc.

  • Plans organizes and facilitates activities as assigned

  • Writes service reports as routine documentation after every session or significant communication with residents

  • Works with residents to accomplish daily life needs as independently as possible. Example: cooking, laundry, hygiene, household routines, etc.

  • Makes recommendations to meet the needs of residents which cannot or should not be provided by the Life Coach: example: medication, therapy by a licensed clinician, private tutor, house cleaner, etc.

  • Communicates with parents and guardians as needed to be sure the resident is receiving the best support possible.

  • Focus all interactions towards helping the residents increase their independence and the quality of their day to day LIFE.

Must have current certification or be willing to attend training through LIFE for CPR/AED/First Aid and any LIFE or DDS required classes.
