We are growing again and are currently seeking 2 more caring and patient individuals to join the LIFE family as Community Direct Care Professionals (DCP) providing instruction, support, and to serve as a positive example to our active young adult clients with learning and cognitive disabilities.

Positions can be located throughout Cape Cod and Plymouth area! Home-base is in Centerville–but vast majority of the position is performed in the community.

To work in this position you must have a positive and flexible mindset and be able to think outside the box.

  • Full-time hours are 11am-7pm Monday-Friday (fully benefit-eligible)
  • Part-time hours are 3pm-7pm Monday-Friday (eligible for PTO, Holidays and Sick time)
  • If you are a match for the position, we will work with you to create a schedule that works for you
 
RESPONSIBILITIES:
  • Assist and maintain the health and safety of the individual.
  • Attend and participate in group social activities.
  • Complete required documentation/daily case notes.
  • Assist with activities of daily living as required.
  • Assist with household chores as required.
  • Assist with menu planning and meal preparation as required.
  • Provide transportation as required.
  • Assist with individual budgeting or spending as needed.
  • Assist the individual with accessing health services as needed.

You will be working in a variety of locations between the mid-and upper Cape Cod (Falmouth to Dennis). A valid driver’s license is required with an acceptable driving record and reliable and insured vehicle (mileage is reimbursed).

Applicant must project a positive and patient personality and provide ongoing support, interaction, and encouragement to our clients. Our staff are always role models to our clients. Applicant must be able to work as a team and independently.

REQUIREMENTS:

  • Must be +18 years old

If you would like either a part-time or full-time position–we can work that out for you. This home has clients who are need a higher level of care–but are not as high energy as some of our other homes. They are waiting to return to all of their activities as the pandemic continues to stabilize.

Candidate must be caring and patient as you will be working with our learning-disabled residents that reside in our lovely Group Home. As this home does have some individuals with physical issues as well, there is an additional $1 per hour differential paid for all hours worked.

The support of these active and heart-warming residents is done through:

• Instruction, guidance and by serving as a positive example to these active adults
• Assisting them to maximize and maintain their independence in their home and in the community by assisting with life skills –cooking, planning, and reviewing daily/weekly schedules, cleaning
• Assisting in preparation for their morning, evening or night routines as needed, etc. and on errands and social activities in the community or virtually.
• Ensuring that the Home role modeled by the staff maintains sanitary conditions of workspaces and vehicles and complies with LIFE guidelines to prevent the spread of COVID-19 (e.g., uses PPE, social distancing, etc.)
• Applicant must be able to project a positive and patient personality and provide ongoing support, interaction, and encouragement to our clients.

REQUIREMENTS:

Fitness Specialist

We are currently seeking a Fitness Specialist who will assist the LIFE Fitness Director and Fitness Team in supporting the community members to reach their fitness and healthy lifestyle goals.

Employee must have:

  • A high degree of patience

Responsibilities include assisting the Fitness Director and Fitness staff with:

  • Working with department staff to track resident’s/participants fitness and nutrition progress
  • Working directly with residents with personal training sessions, fitness assessments and overall promotion of living a healthy lifestyle
  • Assisting in coaching our year-round Multi-sport Special Olympics Program
  • Organizing and executing our Annual 5K Road Race/Walk and other fundraising activities
  • Expansion of our department Brain Gym and mental skills training program
  • Developing positive relationships with LIFE residents, families and staff
  • Strong computer skills
  • Ability to learn a variety of computer platforms

Preferred Background

  • AA or BA in physical education/nutrition or related fields
  • Experience with fitness modalities including training, adaptive physical education, physical therapy or nutrition.
  • Experience with learning disabled population and coaching a big plus. (Personal or volunteer work counts!)
  • Experience in leading and coaching in other environments also key

Job Coach for Day Program (Sandwich, MA)

We are currently seeking a JOB COACH for our CBDS (Community Based Day Services) Program to provide ON-SITE and COMMUNITY counseling, training and individual instruction to our wonderful LIFE residents with learning disabilities to help them reach their full potential in the workplace. This position is based at the Rte 6A Sandwich Day Services location and the surrounding community. Must live on Cape Cod or the surrounding area.

  • Applicant must be patient, outgoing and organized.
  • Have excellent verbal and training skills.
  • Enjoy working with people of different age groups and abilities.
  • Be a team player.
  • Proficiency in basic technology (examples: Microsoft Office, etc.).
  • Have the ability to support learning and cognitively disabled adults to help them build confidence and independence in the workplace.
  • Must provide resident transportation to and from employment sites and will occasionally need to use personal vehicle (will be reimbursed for mileage)
  • Must have the stamina to be present in the varied workplaces (may include being outside) with the resident(s) to provide proper job coaching support.

Position is 35-40 hours weekly and are 8AM-4PM-ish (we can be somewhat flexible).

Flexibility with part-time availability may be available as well.

    Shared Living Provider (Multiple Locations)

    Join our growing team of compassionate and reliable providers to support the wonderful individuals we serve at LIFE.

    Shared Living is an arrangement available to qualified adults that is funded by The Department of Developmental Services (DDS).

    We are seeking a compassionate and reliable Shared Living Provider to oversee care for an individual with DDS services to reside in their home or yours, depending on the circumstances.

    The Shared Living Provider would live with this young person.

    In this crucial role you will:

    • Help our participant to live a full and meaningful life.
    • Be able to provide hands on ADL as needed.
    • Continuously attend to medical needs under the Medication Administration Program (MAP) training provided.
    • Drive and transport participant to appointments and activities.
    • Monitor on-going medical issues.
    • Take vital signs as needed.
    • Assist in planning, food shopping, prep, and cooking with participant.
    • Prepare paperwork for and attend appointments.
    • Transport participant to appointments, and activities.

    Qualifications:

    • MAP Certified (Will provide training)
    • First Aid & CPR Certified (Will provide training)

    The LIFE Community is seeking 1 Full Time and 1 Part Time JOB COACH to provide counseling, training and individual instruction to our wonderful LIFE clients to help them reach their full potential through workplace training in their workplaces that are located throughout the Mid-Upper Cape Cod area.

    • Must live in or around the Cape Cod, MA area.
    • The full-time position is primarily Monday-Friday regular business hours, however Saturday is required. This full-time position (35-40 hours) offers an exceptional benefits package.
    • The Part-time position is shorter day shifts–Saturday is also required for this position.
    • The job coach will work in the Hyannis to Mashpee areas working with different individuals at different work locations to help them be their best!

    • Must be willing to attend training through LIFE for CPR/First Aid (great if you already have) and be willing to complete any LIFE or DDS required/recommended training classes
    • Must have a valid driver’s license with acceptable driving record and dependable transportation; must be able to pass full background check including DPPC, and CORI and fingerprinting background checks per DDS.
