Prescription for Success

In just three years, Julia, a LIFE participant, has worked her way up from the stock room at Apothecare of Cape Cod in Hyannis to training to be a pharmacy technician.

“It’s a really special relationship,” said Julia’s employment coach, Stephanie Cappella. “She started out just doing the packaging, but the management saw a lot of potential in her and wanted to try the training. She’s very independent at work, and they’re so supportive. They love her.”

“This is exactly what you want to happen—having employers believing in our participants and willing to give them a chance,” she continued. “That’s all you can hope for.”

Julia, 24, enjoys the job, especially the accuracy component of it. She also feels gratified that she’s helping people get their prescriptions. “It’s important work,” she said.

Julia works 16 hours a week, commuting on her own from LIFE’s supported independent living community in Hyannis, where she lives with a housemate. She has been living there since 2017 and enjoys the social aspect, going shopping and having weekly dance parties and ladies’ nights.

Maintenance Manager (Hyannis, Mashpee)

LIFE is seeking a Maintenance Manager to oversee day to day safety and maintenance of our Hyannis and Mashpee community and group homes.

The position is approximately 30 hours per week, primarily Monday through Friday. We have flexibility in exactly how that works to make it work for the candidate. Ideal schedule would be from 8:00 AM to 2PM-ish.

This position would be benefits eligible--medical, dental, life/LTD, PTO, holiday time and sick time.

The ideal candidate will have strong organizational and communication skills and broad knowledge of facility maintenance, such as light carpentry, painting, etc., ability to work independently; ability to determine when outside vendors should be utilized; ability to listen and patiently work around our amazing learning/developmentally disabled population. 

Position would include managing landscaping, irrigation systems, snow removal, oversight of maintenance and repair of all LIFE vehicles including registrations, inspections and accident repair.

Plan and organize LIFE annual maintenance inspections (fire sprinkler, fire extinguisher, etc.).

Maintain all standards for DDS (Department of Development Services) such as water testing, furnace and hot water heater inspections.

Participate in safety committee meetings and working with the condominium association and owners.

Available as needed for emergencies.

Direct Service Professional (West Yarmouth)

Part-Time or Full-Time

$15 per hour; $16 per hour on Sundays 9 AM - 9 PM

Direct Service Professionals in Small Group Home (Hyannis)

36 hours a week, $15.00 per hour and $1.00 for all working hours.

LIFE is currently seeking one employee to work three 12-hour overnight shifts on Sunday 9:00 pm to 9:00 am; Monday 9:00 pm to 9:00 am and Friday 9:00 pm to 9:00 am.

Candidate must be caring and patient as this home works with our Aging in Place cognitively disabled adults in their beautiful Home in Hyannis. This home has four clients in residence. You will never find a nicer group of individuals to support.

This particular home does require some additional hands on with the residents in their personal care and ADL’s and therefore offers an additional $1.00 per hour for all hours worked. Prior PCA or CNA education or experience great, but not required.

Community Direct Care Professional (Mid & Upper Cape Cod)

Shared Living Provider (Multiple Locations)

Join our growing team of compassionate and reliable providers to support the wonderful individuals we serve at LIFE.

Shared Living is an arrangement available to qualified adults that is funded by The Department of Developmental Services (DDS).

We are seeking a compassionate and reliable Shared Living Provider to oversee care for an individual with DDS services to reside in their home or yours, depending on the circumstances.

The Shared Living Provider would live with this young person.

In this crucial role you will:

• Help our participant to live a full and meaningful life.
• Be able to provide hands on ADL as needed.
• Continuously attend to medical needs under the Medication Administration Program (MAP) training provided.
• Drive and transport participant to appointments and activities.
• Monitor on-going medical issues.
• Take vital signs as needed.
• Assist in planning, food shopping, prep, and cooking with participant.
• Prepare paperwork for and attend appointments.
• Transport participant to appointments, and activities.

Qualifications:

• MAP Certified (Will provide training)
• First Aid & CPR Certified (Will provide training)
• Valid Driver’s License w/ reliable transportation (vehicle)
• COVID guidelines followed – PPE, etc. as required.

Job Coach (Multiple Locations: Cape Cod, Plymouth, MA)

Living Independently Forever, Inc. (LIFE) communities based on Cape Cod and in Plymouth, MA is seeking two Job Coaches to provide counseling, training, and individual instruction to help our wonderful LIFE clients with cognitive and learning disabilities reach their full potential through workplace training. 

Employment can be full-time or part-time. The position is primarily Monday through Friday business hours, but will include one weekend per month and occasional evenings until 7:00 pm. 

Applicant must:

• Be patient and kind 

• Be outgoing and organized

• Have excellent verbal and training skills; some computer skills needed

• Enjoy working with people of different age groups, cultures and abilities 

• Be a team player

• Be able to build confidence and independence in our clients in the workplace 

• Provide resident transportation to and from employment sites as needed. Employee will need to use personal vehicle much of the time (employees are reimbursed for mileage) 

• Must have the stamina to be present in the varied workplaces with the client to provide proper job coaching support. 

• Maintain sanitary conditions of workspaces and vehicles; continually works with clients to encourage their understanding and compliance with LIFE and their employer's guidelines to prevent the spread of COVID-19 (e.g., must wear mask, observe social distancing, etc.)

Job Coach - Case and Money Manager (Mashpee)

LIFE is seeking a Full Time Case Manager/LIFE Coach to assist our wonderful residents with learning and cognitive disabilities to fulfill their independent potential by supporting them in managing their daily life skills (money management, household management, shopping, menu planning, cooking, personal presentation and hygiene, appropriate behavioral and social skills, travel arrangements, transportation needs, medical appointments, entertainment, etc.).

Full-time position with 40 hours primarily performed during normal business hours (8:00 a.m.-6:00 p.m.), but may occasionally require a later evening or weekend.

Applicant must be patient, vibrant, energetic and highly organized with excellent time management skills. Must possess excellent verbal and written communication skills; must possess excellent money management skills, strong computer skills; and the ability to multi- task. Must be a team player!

- Experience with learning disabled population required.
- Social service work experience highly desired.
- Bachelor's degree or 3+ years of directly related experience required.

Responsibilities include:
- Assist assigned residents with the management of their bills and finances--overall money management.
- Assist assigned residents with medical appointments and medication organization and proper documentation.
- Meets individually with assigned residents, provide assessment of strengths and needs in all areas, and writes appropriate objectives and goals (ISP's).
- Assures adequate attention is given to skill and growth in all aspects of the resident's life and helps prioritize areas of emphasis in conjunction with the Employment Team.
- Participates in team-related services; DDS meetings, staff meetings, housemate issues, case reviews with staff and/or family etc.
- Plans, organizes and facilitates resident activity two evenings weekly.
- Role models appropriate behavior and encourages residents in leading a healthy and active lifestyle.
- Writes service reports as routine documentation after every session or significant communication with residents.
- Makes recommendations to meet needs of residents which cannot or should not be provided by the Life Coach: example: medication, therapy by licensed clinician, private tutor, house cleaner, etc.

Focus all interactions towards helping the residents increase their independence and the quality of their day-to-day LIFE.

Must have current certification or be willing to attend training through LIFE for CPR/AED/FirstAid and any LIFE or DDS required/recommended training classes.

Valid driver's license required with acceptable driving record and dependable transportation; must be able to pass CORI and fingerprinting as per DDS.

Employment Counselor (Plymouth)

The LIFE Community based out of The Pinehills in Plymouth, Massachusetts is seeking a full time Employment Counselor to work with our exceptional residents, community employers and employment staff to locate employment/volunteer opportunities and to match the right individual to the right position. 

The position is Monday-Friday regular business hours. We have flexibility in exactly how that works to make it work for the candidate.  After training, SOME of the position may be performed remotely. The position offers an exceptional benefits package.
