Supported Independent Living

LIFE serves over 100 individuals in Supported Independent Living (SIL) communities in Hyannis, Mashpee and Plymouth. Each community is in a safe neighborhood, with access to public transportation, job opportunities, and fun things to do. SIL participants live on their own or with a roommate and receive between 2 to 15 hours of support a week. Each community features a common area, fitness facilities, weekly social activities, and emergency on-call staff.

Candidates for a SIL community are at least 19 years of age, have a history of intellectual disability or autism, demonstrate independent living skills, such as home-alone safety, the ability to cook, clean, manage money and to self-medicate with minimal support.

The successful SIL individual will be involved in a work or volunteer job and participate in LIFE social and fitness activities. Our professional staff will help to develop independent living skills and a healthy lifestyle. We do this in partnership with families while honoring the human rights of the adults we serve.

The SIL community is not appropriate for individuals with a history of illegal behavior, alcohol/ substance abuse, self-injurious or aggressive behavior.

Shared living residence

Shared Living

Shared Living provides a supported living environment within a private home of a family, couple or individual. Shared Living Providers offer safety, comfort, and supports in the areas of meal preparation, transportation, socialization, and medication administration. Shared Living offers the individual with a caring, home-like environment, and the provider the opportunity for meaningful work in a non-traditional setting.

LIFE offers a network of Shared Living participants and providers, as well as access to some of LIFE’s social activities and communities.

LIFE Coaches

Group and Hybrid Homes

For individuals requiring additional supports, LIFE offers beautifully appointed Group Homes in safe neighborhoods located throughout Cape Cod. Individuals in LIFE Group Homes receive 24/7 staffing supports, access to the LIFE community and facilities, and a social calendar that takes advantage of all the wonderful things New England has to offer. Each home features spacious bedrooms and common areas. Nursing and clinical support are also available. Staff support Group Home individuals toward greater independence in all areas of life.

Hybrid homes offer supports for those individuals needing more than what SIL provides, but less than 24/7 care. A successful Hybrid candidate will have some home-alone safety skills, be able to self-medicate, participate in a day program or work, and thrive with 40 or fewer hours of support a week.

Our goal is to work with participants on developing the necessary skills to function as independently as possible within the community. By working within the circle of LIFE commitment with LIFE coaching, Employment, Healthy LIFEstyle, and Social LIFE, we try to achieve a complete way of LIFE.

 

LIFE Coaches

LIFE Coaches and Direct Care Staff

LIFE staff provides support in all areas of independent living. Assistance and education are provided around healthy meal planning, grocery shopping, and money management practices. Staff will assist with the planning of annual medical appointments and transportation needs as well. Participants will receive support for social engagement and healthy relationships. We tailor supports to the unique needs of each person.

A fulfilling job is an important component of living a meaningful life. It provides a sense of purpose while offering a much-needed routine and structure. At LIFE we believe that everyone deserves the chance to answer the question, “So what do you do for a living?”

LIFE Coaches

Healthy LIFEstyle

An important component of the LIFE community is the fitness and wellness program. Each community has its own fitness center with state-of-the-art equipment, as well as large screen TVs and sound systems to enhance workouts. LIFE employs a full-time certified fitness staff to facilitate a wide variety of recreational activities and fitness challenges.

Individuals are invited to participate in sports and games. Many enjoy walking on local beaches, biking, rollerblading, basketball and golf. LIFE also offers the largest and most successful Special Olympics program in the state. We have a lot of fun while getting (and staying) in shape!

LIFE also offers nutrition programs and cooking classes to encourage healthy diets. Residents gather frequently in the recreation centers to socialize and prepare meals. At LIFE, we believe that a healthy lifestyle leads to a greater sense of well-being as well as to a happier life.

Social LIFE

At LIFE, there is an activity almost every day of the week, ranging from Men’s and Ladies’ clubs, Rec Night, Dance Parties, and more. Making and maintaining friendships is important, and these group activities create an ideal environment for social interaction.

LIFE participants take advantage of local offerings including theater, sporting events, bowling, beaches, and restaurants. Group trips have included Martha’s Vineyard, Disney World, New York City, and Washington DC. There is always something to do at LIFE!
