You can start the LIFE admissions process here.
The decision to enroll in a supported services community program for adults with autism, learning and intellectual disabilities is a significant one, and the LIFE application process reflects this.
You will need to upload documents, including reports from recent programs, current physical records from your family physician, a cognitive evaluation, and additional assessments.
If you are not ready to complete our formal application at this time, we invite you to connect with Rachelle Jones, Director of Admissions, via her contact information listed below, to learn more about LIFE programs and discuss what’s the best fit for your future applicant.
Admissions decisions are made based on the compatibility fit between the applicant and the program.
Are you considering seeking DDS funding? If yes, before you begin the LIFE admission process, please submit an application to DDS using the process described on the Mass.gov website.

Admissions FAQs
LIFE offers a variety of programs for individuals, including residential and community programs, each with different levels of support.
We invite you to explore our website to explore our different programs and then schedule a conversation with Rachelle Jonas, LIFE Admissions Director, to talk about your family member and learn about the all the wonderful opportunities provided by LIFE.
Based on extensive experience and knowledge of LIFE’s communities, Rachelle will provide LIFE program recommendations that best matches your family member’s strength, interests and abilities.
As a guideline, these frequently asked questions can provide an overview of the admissions process.
What’s the application process for the LIFE Supported Independent Living (SIL) program?
Once, all your application materials are received, LIFE will contact you to schedule an assessment.
What’s the cost to apply?
The application fee is $450 which can be paid by check made paid payable to LIFE Inc.
What happens after I submit an application?
After you submit an application to the appropriate program (SIL, shared living), then there will be some subsequent documentation required.

What programs and supports does LIFE offer?
LIFE offers a mix of residential programs and community-based programs. We invite to explore these programs.
Does LIFE accept DDS funding?
LIFE recommends you start with a conversation with your local Turning 22 program at DDS to learn about adult services and funding.
Is there a difference between the SIL community locations?
Yes, the LIFE Plymouth community is an integrated community within the greater community.
The LIFE Mashpee and Hyannis communities have separate residential communities for only LIFE residents.
Do I have to buy a condo?
No!
LIFE Hyannis and Mashpee campuses has LIFE owned condos where families can rent a two bedrooms unit, depending on availability.
LIFE Plymouth has various apartment options where individuals and family members rent through the building leasing offices at HarborWalk, Oasis, and Pinehills for studio, one or two bedroom options.
